Job Summary: We are seeking an experienced Director of Nursing (DON) to lead clinical operations and ensure the delivery of safe, high-quality, and compliant home health care. The DON will oversee clinical staff, patient care plans, scheduling, documentation, regulatory compliance, quality improvement, infection control, staff training, and interdisciplinary coordination, while supporting recruitment, onboarding, performance management, and policy development. The ideal candidate is a graduate of an accredited nursing program with a current Illinois RN license, strong leadership and communication skills, and knowledge of home health regulations and accreditation standards. Job Summary

• The Director of Nursing (DON) supports the Administrator in overseeing the delivery of home health services to patients in their residences.

• The DON is responsible for ensuring the provision of high-quality, safe, and effective clinical care in accordance with regulatory and agency standards.

• The DON (or qualified designee) must be available during operating hours and actively participates in all professional service activities, including personnel oversight, policy development, and quality improvement initiatives.

Qualifications

• Graduate of an accredited school of professional nursing; currently licensed as a Registered Nurse in the State of Illinois, in good standing.

• Strong leadership, organizational, communication, and problem-solving skills.

• Working knowledge of home health, state/federal guidelines, and accreditation standards.

• Ability to exercise independent judgment and promote team collaboration.

Essential Responsibilities

• Ensure compliance with all applicable laws, regulations, and agency policies.

• Develop and enforce clinical standards for safe and effective patient care.

• Supervise clinical staff and conduct training, evaluation and competency.

• Oversee clinical scheduling, visit assignments, and interdisciplinary coordination.

• Lead recruitment, hiring, onboarding, and disciplinary processes.

• Organize and lead clinical meetings and staff education.

• Oversee development and review of individualized patient care plans.

• Ensure timely, accurate, and compliant documentation.

• Coordinate care with physicians and community partners.

• Serve as Infection Control Officer.

• Monitor medical records for clinical and regulatory compliance.

• Maintain clinical staff availability during designated operating hours.

• Participate in policy development, strategic planning, and administrative initiatives.

• Address incidents, concerns, and complaints in a timely manner.

• Act as Administrator in their absence.

• Engage in community outreach as needed.

• Maintain confidentiality of patient and agency information. 18. Fulfill additional duties as assigned (i.e., field visits)

Working Conditions

• Work is performed in an office environment with required travel to patient homes, as needed.

• Exposure to high-risk clinical situations.

• Must be able to lift up to 50 lbs and perform clinical tasks as needed.

• On-call responsibilities as assigned.
