Job Summary: <p>Seeking a licensed Physical Therapist to provide skilled, patient-centered therapy services in patients’ homes. The PT will independently manage evaluations, treatment plans, functional rehabilitation, patient/caregiver education, and clinical documentation while collaborating with an interdisciplinary care team.</p>

Position Overview

We are seeking a compassionate and motivated Physical Therapist (PT) to provide skilled physical therapy services to patients in their homes throughout Waterbury and surrounding communities.

The ideal candidate will be comfortable working independently, building strong relationships with patients and caregivers, and collaborating with an interdisciplinary clinical team. This role offers the opportunity to provide meaningful patient-centered care while contributing to the growth and development of a home health therapy program.

Key Responsibilities

  • Perform comprehensive physical therapy evaluations and reassessments.

  • Develop individualized plans of care based on patient needs, functional status, and goals.

  • Provide skilled physical therapy interventions focused on:

    • Mobility and ambulation

    • Strength and endurance

    • Balance and coordination

    • Transfers

    • Fall prevention and safety

    • Functional independence

  • Educate patients, families, and caregivers regarding therapeutic exercises, equipment use, mobility strategies, and home safety.

  • Coordinate patient care with nursing, occupational therapy, physicians, case managers, caregivers, and other healthcare providers.

  • Participate in interdisciplinary communication and care planning.

  • Monitor patient progress and identify changes in condition.

  • Communicate significant clinical changes or concerns promptly to the appropriate members of the care team.

  • Complete accurate, timely, and compliant clinical documentation.

  • Promote safe patient function and help patients remain independent in their homes.

  • Participate in therapy program development, mentoring, and other clinical initiatives as applicable.

Qualifications

  • Current Connecticut Physical Therapist license.

  • Graduate of an accredited Physical Therapy program.

  • Valid driver's license and reliable transportation.

  • Strong clinical, communication, organizational, and documentation skills.

  • Ability to work independently in a home-based environment.

  • Ability to effectively collaborate with an interdisciplinary healthcare team.

  • Positive attitude and strong commitment to patient-centered care.

Preferred Experience

  • Previous home health physical therapy experience preferred.

  • Candidates with strong clinical experience in other settings who are interested in transitioning to home health are encouraged to apply.
